One-Time $100 Plan Setup Fee
(enroll as many members as you'd like. Pay only one Setup Fee).

Each Adult: $25 per Month
Each Child: $20 per Month

By enrolling in the Garden State Smiles savings plan, you acknowledge and agree that you are entering into a one year agreement. You agree that your credit or debit will be billed automatically each month, and will automatically continue each year until cancelled. If you wish to cancel, you must contact us 30 days before your yearly anniversary.
